Complete the Current Release/Waiver & Code of Conduct
** Completion of the Code of Conduct & Waiver is required annually by all CCHfH Volunteers**
–In addition to the Code of Conduct & Waiver, the Volunteer Waiver Minor Supplement must be completed by a parent or legal guardian for any volunteer who is under the age of 18.

IMPORTANT INSTRUCTIONS TO COMPLETE ELECTRONIC FORM:
* Right-click on the links above, left-click on save link as, then rename your file and save it to your desktop. Be sure to include your first initial & last name in the file name, the form title, and today’s date (Example: FLast-Waiver_Mar 5 2023).
* After the file downloads, click the up-arrow at the bottom left of your screen & select open with system viewer. Fill in the fields on the form, then click the save button at the top of the page & email to coordinator@chaffeehabitat.org.
